SAN FRANCISCO, CA – A new clinical trial shows that a two-stent technique for treatment of bifurcation lesions with a large stenotic side branch was not associated with significant improved outcomes compared to a provisional stenting approach. The Nordic-Baltic Bifurcation Study IV - No Study Results Posted. The Nordic Baltic Bifurcation Study IV (Nordic Baltic IV) was a prospective, randomised, multicentre trial comparing the simple provisional SB stenting technique versus complex stenting of both the MV and the SB in the treatment of coronary bifurcation lesions.

The randomized Nordic Baltic bifurcation study IV was first to show favorable mid-term results for two-stent techniques compared to provisional side branch stenting. Two-stent techniques have been asso-ciated with more stent strut malapposition compared to main vessel only stenting. Here we present the Nordic Baltic IV OCT substudy.

The Nordic–Baltic Bifurcation IV study in true bifurcation lesions showed consistent results, albeit without reaching statistical significance. 6 Based on these studies, in particular DKCRUSH-V, recent guidelines opened the door for a systematic two-stent approach in left main bifurcation lesions. 1

Methods: The study was a randomised, superiority trial. Enrolment required a SB≥2.75 mm, ≥50% diameter stenosis in both vessels, and allowed SB lesion length up to 15 mm. The primary endpoint was a composite of cardiac death, non-procedural myocardial infarction and target lesion revascularisation at 6 months.
Provisional SB stening n221. Two stent n229. 1 lost to FU 1 excluded due to protocol violation. 1 withdrawal.

Sponsored by the Cardiovascular Research Foundation (CRF), TCT is the world’s premier educational meeting specializing in interventional cardiovascular medicine. The Nordic Baltic Bifurcation Study IV (Nordic Baltic IV) was a prospective, randomised, multicentre trial comparing the simple provisional SB stenting technique versus complex stenting of both the MV and the SB in the treatment of coronary bifurcation lesions. The Nordic-Baltic Bifurcation Study IV (BIF IV) and the European Bifurcation Club (EBC) TWO trial have both been designed to answer this question. Both studies compare simple versus complex stenting in larger true bifurcations with side branch diameters being at least 2.5 mm.” The randomized Nordic Baltic bifurcation study IV was first to show favorable mid-term results for two-stent techniques compared to provisional side branch stenting. Two-stent techniques have been asso-ciated with more stent strut malapposition compared to main vessel only stenting.
